الوصف: Purpose: To evaluate outcomes of slow coagulation transscleral cyclophotocoagulation (SC-TSCPC) in a primarily African American patient population with glaucoma. Methods: A retrospective chart review was performed for 104 consecutive cases of SC-TSCPC by a single surgeon between November 6, 2019–September 7, 2023. Power ranged from 1150 to 1500 mW, duration was 4 s, and number of spots ranged from 10 to 25. Exclusion criteria were diagnosis of neovascular glaucoma, prior CPC, visual acuity (VA) of no light perception or unable to be assessed due to patient's mental status, aphakia, or follow-up

الوصف: Purpose: Iris pigment deficiency in patients with oculocutaneous albinism (OCA) often causes debilitating photophobia, which is routinely managed by sequential intracapsular insertion of two aniridia rings. A common concern of this technique is the risk of segment interdigitation, which can lead to intraoperative complications. Observations: In this report, we describe a modified technique through which both rings were stacked together and inserted simultaneously in a 62-year-old male patient with oculocutaneous albinism and a mild cataract. The patient underwent bilateral phacoemulsification followed by implantation of two Morcher 50E aniridia rings simultaneously, prior to IOL implantation behind the implants within the capsular bag. Post-operatively, the patient's photophobia improved dramatically, and there were no post-operative complications. Conclusions and Importance: Simultaneous insertion of two stacked aniridia rings prior to IOL implantation was a safe and successful alternative surgical method for the management of photophobia in patients with oculocutaneous albinism and cataracts.

الوصف: Purpose: To describe the outcomes of a novel technique for the repair of peripheral corneal perforations using autologous limbal tissue. Methods: Two patients with peripheral corneal perforations with contraindications to other corneal procedures underwent limbal advancements. This technique involves creating a pedicle of the limbus, sclera and conjunctiva to cover the perforation. Results: The tissue had optimal integration in both patients; no aqueous leaks or flat anterior chambers were noted. None of the patients had recurrence of perforation or ocular discomfort. Conclusion: In conclusion, this technique is a cost-effective and straightforward alternative for the repair of impending acute peripheral perforations.

الوصف: Purpose: To retrospectively report a case of complex retinal detachment secondary to an intraocular episcleral hydrogel explant (MIRAgel, MIRA Inc., Waltham, MA, USA) invasion. Observations: The severe vitreous inflammation, shifting nature of the subretinal fluid, and absence of proliferative vitreopathy 2 months following multiple surgeries for removal of fragmented MIRA gel in the present case made the diagnosis of complex retinal detachment. With the assistance of the fragmatome and Perfluorocarbon, the subretinal MIRA gel, and turbid, yellowish subretinal fluid were removed successfully. The retina was re-attached without recurrence. Conclusions and importance: By using the pars plana ultrasonic fragmatome, the intraocular MIRAgel was able to be removed while maintaining the integrity of the retina and globe.

الوصف: Purpose: To describe a novel surgical technique for the removal of a live intravitreal nematode. Observations: We describe the surgical management of a 35-year-old man with a live intravitreal nematode in his left eye. A 23-gauge pars plana limited core vitrectomy was performed to release the nematode from the surrounding vitreous strands. After creating a peritomy, a 20-gauge full thickness sclerotomy was created with a microvitreoretinal blade. The tip of a 20-gauge angiocath needle was cut obliquely and inserted through the sclerotomy. The external portion of the needle was attached to the viscous fluid extraction tubing for the Constellation vitrectomy machine (Alcon). Gentle aspiration pulled the nematode into the angiocath. The nematode was transferred to a syringe for parasitologic evaluation. Conclusions and Importance: The novel surgical technique described allows for aspiration of an intravitreal nematode in a controlled fashion. This technique can be used to manage this rare, but visually significant condition.

الوصف: Purpose: to present the now vitrectomy technique through a limbus via preexisting holes in the Eckardt TKP. Methods: the surgical technique performed in a case of four-year-old girl presented with a complete hyphema and vitreous hemorrhage in the left eye. Results: the vitrectomy was performed vial a limbal approach from the TKP without making additional sclerotomies. The vitreous hemorrhage was evacuated, the hyaloid elevated, the periphery examined with an excellent widefield view without scleral depression, an air-fluid exchange performed, and the air exchanged for silicone oil. Finally, the TKP was replaced with a permanent graft. The surgical video was presented additionally. Conclusion and importance: Eckardt TKP allows for a limbal approach in aphakic or non-lens sparing vitrectomy surgery while maintaining a stable intraocular pressure, excellent peripheral and posterior access and while avoiding the need for pars plana sclerotomy placement which can be challenging in complex cases.
