With an estimated average of six million unemployed over the past year, and the recent uptick in unemployment due to the COVID-19 pandemic, it is important to identify methods to mitigate and reduce the negative health outcomes often associated with under- and unemployment. Objectives We aimed to 1) outline the process of partnering with a community organization to implement a modified MBSR program for under- and unemployed individuals, and 2) present pilot data on preliminary results. Methods The modified MBSR program was implemented in two phases within a job training program for under- and unemployed individuals. In Phase I, group one received an eight-week program. Based on feedback, the MBSR program was reduced to six weeks and implemented for groups two and three (Phase II). Feasibility and acceptability were evaluated utilizing a mixed-methods approach. Changes in mindfulness, perceived stress, pain interference, anxiety, depression, and sleep disturbance were assessed pre-post the modified MBSR program. Results Thirty-three participants completed the program with twenty-nine post-survey responses. The modified MBSR program was feasible and acceptable as evidenced by the enrollment rate (96%), retention rate (72%), and qualitative feedback. Fifty-percent of participants self-reported weekly home practice compliance. Perceived stress and mindfulness demonstrated significant moderate improvements ( d = .69, p = .005; d = .46, p = .001). Depression, anxiety, and pain interference results suggested small non-significant effect size improvements ( d = .27, p = .19; d = .23, p = .31; d = .25, p = .07). Effects on fatigue and sleep disturbance were negligible. Conclusion The modified MBSR program was feasible and acceptable to the organization and participants. Small to moderate improvements in mental health and pain interference outcomes were observed. Research using larger sample sizes and randomized designs is warranted.

The success seen with medical students at this university led to great interest in expanding the mind-body skills program so students in other disciplines could benefit from the program. Objective The purpose of this study was to assess the effects of a 9-week mind-body skills program on the mental and emotional well-being of multidisciplinary students compared to controls. We also sought to determine if the program’s effects were sustained at 1-year follow-up. Methods A cross-sectional pre-post survey was administered online via SurveyMonkey to participants of a 9-week mind-body skills program and a control group of students from 7 colleges at a public university from 2017–2019. Students were assessed on validated measures of stress, positive/negative affect, resilience, depression, anxiety, fatigue, sleep disturbance, mindfulness, empathy, and burnout. Scores were analyzed between-groups and within-groups using bivariate and multivariate analyses. A 1-year follow-up was completed on a subset of participants and controls. Results 279 participants and 247 controls completed the pre-survey and post-survey (79% response rate; 71% female, 68% white, mean age = 25 years). Participants showed significant decreases in stress, negative affect, depression, anxiety, sleep disturbance, and burnout, while positive affect, resilience, mindfulness, and empathy increased significantly ( P < .05). Only sleep disturbance showed a significant decrease in the control group. Follow-up in a subset of participants showed that only mindfulness remained elevated at 1-year ( P < .05), whereas the significant changes in other well-being measures were not sustained. Conclusion Participation in a 9-week mind-body skills program led to significant improvement in indicators of well-being in multidisciplinary students. A pilot 1-year follow-up suggests that effects are only sustained for mindfulness, but not other parameters. Future programming should focus on implementing mind-body skills booster sessions to help sustain the well-being benefits.

It encompasses a wide range of interventions that are similar yet different in meaningful ways. Mindfulness and relaxation practices are 2 mind–body techniques that have similarities and differences; however, these techniques are often used or discussed interchangeably, such that the differences between them become obscured. A greater understanding of the unique facets of mindfulness and relaxation is needed for researchers and clinicians to make informed decisions when selecting an approach. The purpose of the current article is to offer an evidence-informed perspective on similarities and differences between mindfulness and relaxation. Specifically, mindfulness and relaxation practices are compared and contrasted in terms of theoretical foundation, intention, and psychological and physiological effects and mechanisms. Implications for clinical practice and suggestions for future research are discussed.

    الوصف: Pain management is a frequent problem in the neonatal intensive care unit (NICU). Few studies examining effects of integrative care therapies on pain-related outcomes in neonates have included physiological outcomes or investigated the use of such therapies in a practice-based setting.The purpose of this practice-based retrospective study was to examine the associations between integrative care therapies, particularly massage and healing touch, and pain-related outcomes among hospitalized infants.We conducted a retrospective review of a clinical database from a level III NICU regularly delivering integrative care therapies. Paired-samples t-tests were used to examine associations between integrative care therapies and 4 pre-post outcome measures: therapist-rated pain and presentation (ranging from asleep to agitated) and neonates' heart rate and oxygen saturation.Of 186 patients (M age=68 days), 58% were male and 67% were Caucasian. Sixty-two percent received both massage and healing touch; the remainder received a single therapy. Pocos de los estudios que examinan los efectos de los tratamientos integradores sobre los resultados relacionados con el dolor en neonatos han incluido los resultados fisiológicos o investigado el uso de este tipo de tratamientos en un entorno basado en la práctica.El objetivo de este estudio retrospectivo basado en la práctica fue examinar la asociación entre los tratamientos integradores de atención, especialmente el masaje y el toque sanador, y los resultados relacionados con el dolor entre los lactantes hospitalizados.Realizamos una revisión retrospectiva de una base de datos clínica de una UCIN de nivel III que proporcionaba regularmente tratamientos integradores de atención. Se utilizaron pruebas t de muestras pareadas para examinar las asociaciones entre los tratamientos integradores de atención y cuatro medidas de resultados anteriores y posteriores: dolor puntuado por el terapeuta y presentación (desde dormido hasta sueño agitado) y frecuencia cardíaca y saturación de oxígeno de los neonatos.De 186 pacientes (Medad = 68 días), el 58 % eran hombres y el 67 % eran de raza caucásica. El 62 % recibió tanto masajes como toque sanador; el resto recibió un solo tratamiento.     الوصف: Focus Areas: Integrative Approaches to Care, Supporting Behavioral Change Diabetes affects approximately 26 million adults and is a leading cause of death and disability in the United States (CDC, 2011). Diabetes disproportionately affects individuals from lower socioeconomic status groups, the same individuals with limited access to healthcare resources (CDC, 2011). Thus, there is a critical need to provide affordable and effective care to these at-risk individuals. In order to meet this need, we developed and implemented a unique integrative medicine (IM) “Shared Medical Visit” approach to diabetes management at a Free Health Clinic. This initiative uses IM techniques and tenants to improve the health and well-being of low-income men and women with Type II diabetes mellitus. The Shared Medical Visit model allows patients to provide and receive social support and learn from others' experiences while receiving high-quality medical care. Visits occur monthly and include 8 to 15 patients, 1 IM physician, and 1 Clinic Primary Care Physician (PCP). The IM physician leads the 90-minute group while the PCP provides individual, private visits as needed. The IM physician's goals are to promote healthy group dynamics, respect, and healthy behavior change. The visits provide several interventions, including nutrition education (eg, whole foods), healthy cooking demonstrations, and food tastings; mind-body stress reduction exercises (eg, mindfulness, breath-work); and physical activity (eg, walking groups). While we did not formally assess outcomes, group members subjectively reported an increased sense of community, appreciation of food tastings, and overall health improvement. One participant reported improved hemoglobin A1C, less medication use, and understanding how stress and sleep contribute to glucose control. The nurse manager reported that participants who attend these visits keep their clinic appointments more often than those who do not. Groups are consistently filled and have been running continuously for 21 months. Plans are underway to formally assess the impact of the group visits on health outcomes.

    الوصف: Focus Area: Sustainable Business Models In 2012, with solely institutional support, the University of Cincinnati officially launched its Center for Integrative Health and Wellness, focused on developing integrative medicine clinical, research, and education initiatives horizontally across the academic health center. Given that Integrative Medicine (IM) is intrinsically interdisciplinary, the UC Center purposively functions according to a horizontal integration business and clinical model, rather than as a siloed vertical structure within one specific clinical or academic area. As opposed to building a separate freestanding physical clinical site, the clinical services are co-located within existing Centers for Excellence and established clinical programs including, for example, a new 25 000–squarefoot Women's Health Center, our Cancer Center, and a Neuroscience Center in development. With its own cost center, IM maintains financial independence while capitalizing on cross-programmatic marketing, development, and infrastructure. Faculty members across the academic health center were engaged early on in the development and planning of the Center in order to inform clinical services tailored to their specific patient populations. The Director of the IM program systematically delivered Grand Rounds across Departments in the College of Medicine in order to market, promote, and plan the IM expansion across the medical school. With an embedded horizontal model of clinical integration, both the financial risks and the successes of the IM program are collaborative and intrinsically shared for optimal sustainability. The Center's future plan for sustainability involves: (1) revenue-generating IM consults and shared medical visits billable through insurance; (2) trainings and wellness educational events; (3) philanthropic support and endowments; (4) federally funded research and education grants; (5) close partnering with the established Integrative Care Department at Cincinnati Children's Hospital Medical Center; and (6) continuing to build strategic horizontal partnerships.
