مستخلص: Objectives: Restrictive spirometric pattern (RSP) has a prevalence of 5.4−9.2% and is associated with various respiratory symptoms, comorbidities, and increased mortality. Breastfeeding has important effects on maternal health; however, the effects of breastfeeding on pulmonary function remain unclear. This study aimed to investigate the effects of breastfeeding on maternal pulmonary function, particularly the risk of RSP. Methods: Retrospective, cross-sectional observational study enrolling parous women aged >40 years who participated in the Korea National Health and Nutrition Examination Survey from 2013−2018. RSP was defined using the FEV1/FVC ratio and FVC outcomes of the pulmonary function test. The adjusted odds ratios (OR) for RSP were calculated using multivariate logistic regression. Results: Of 9261 parous women, 913 (9.9%) had RSP. Breastfeeding (≥1 month) was associated with a reduced risk of RSP (OR: 0.75 [0.60−0.92]) when adjusted for age, body mass index, smoking status, other diseases, socioeconomic status, and maternal risk factors. The adjusted ORs for RSP for women decreased further with increasing duration of breastfeeding (p for trend: 0.0004). The FEV1, FVC, and FVC% were higher in women who breastfed than in those who did not breastfeed (by 0.0390 L, 0.0521 L, 0.9540% p, respectively). Conclusions: There is an association between breastfeeding and pulmonary function in parous women. Breastfeeding was associated with a lower prevalence of RSP in parous women aged >40 years old, suggesting that breastfeeding may have a beneficial effect on maternal pulmonary function.

مستخلص: Building one's own identity and finding meaning in life are important tasks for emerging adults. Although many agree that both are essential in terms of the well-being and proper functioning of an individual, there is little empirical data concerning the relationship between the way young people process information about themselves and the feeling that their life has a purpose and value. Therefore, the aim of this study was to explore how identity processing styles (normative, diffuse-avoidant and informational) and two dimensions of meaning of life (presence and the search for meaning) are related. It was also assumed that identity commitment could play a mediating role between those variables. 183 emerging adults who participated in the study completed Identity Style Inventory (ISI-5) and Meaning in Life Questionnaire. The cross-sectional study design was used. Using Structural Equation Modeling revealed that commitment mediated the relationship between two of three identity styles (diffuse-avoidant and informational) and presence, but not search for or meaning in life. Normative identity style also negatively predicted the search for meaning in life. It seems that commitment might play an important role in meaning-making process, and as such, should be strengthened and encouraged when working with individuals at this stage in life.

مستخلص: Global changes require urgent integration of health and wellbeing into all urban policies. Complex social and environmental factors define wellbeing outcomes and inequities present in cities. Additionally, political decisions are seldom thought and developed considering the needs and participation of children and adolescents. The REDibuja study aims to develop a multidimensional framework of wellbeing for children and adolescents and to validate an index of opportunities for better wellbeing for children and adolescents in the urban context of Temuco, Chile. This child-centered and cross-sectional study will involve mixed methodologies throughout the implementation of five work packages for two years (2022-2023): (1) development of a conceptual framework for child and adolescent wellbeing, (2) integration of available and public data, (3) studies in the local context, (4) data integration using geographic information systems, and (5) validation of the wellbeing opportunity index for children and adolescents. REDibuja will implement methodologies that until now are little used to facilitate political decisions in our regional context. This process and results could be transferred for assessment and decision-making in Latin America and low- and middle-income countries in other regions.

مستخلص: The absence of scope of practice guidelines may lead to role ambiguity and legal consequences in nursing practice. This study measures the scope of practice of nurses in Saudi Arabia. The study utilized a descriptive cross-sectional design using an electronic version of the Arabic Actual Scope of Nursing Practice (A-ASCOP) questionnaire among 928 nurses. Descriptive analysis was followed by a t-test and an analysis of variance (ANOVA). Significance was assured through the Bonferroni test; the effect size was measured through partial η2 when appropriate. The A-ASCOP mean score of each dimension ranged from 4.29 to 4.72 (overall mean = 4.59). Significant overall ASCOP score variations were evident, with higher ASCOP among expatriate nurses, females, Hospital Operation Program (HOP) nurses, and nurses with postgraduate qualifications. Partial η2 showed a small effect of <0.016. Low-complexity nursing tasks showed insignificant differences no matter the nurse’s position, but were less practiced by Bachelor of Science in Nursing (BSN) and advanced-degree nurses than by those with a diploma education. High complexity of ASCOP was practiced significantly more often by postgraduate-prepared nurses than by diploma-educated nurses. The study showed that there is a range of variation in nursing practice, but that the lack of internal regulations (nursing scope of practice) has no effect on nursing duties. In a country such as Saudi Arabia, where massive national improvement initiatives are frequent, clearly defining the scope of practice for nurses is essential and needs to be done through government mandates. Further studies are essential to define what the scope of practice should include.

مستخلص: Objective: This study used a long-term trend analysis to investigate whether gender differences were related to the risk of injury and epidemiological characteristics in Taiwan from 1998 to 2015. Materials and methods: Data on 4,647,259 hospitalized patients that were injured from 1 January 1998, to 31 December 2015 were collected from the National Health Insurance Research Database (NHIRD). Among the injured patients, 2,721,612 males and 1,925,446 females were identified. Patients were age-, gender-, and index date-matched. Multiple logistic regression was used to analyze the risks of injury via gender differences. A p-value < 0.05 was considered significant. Results: The injury risk of the male patients was 1.4 times higher than that of female patients (AOR = 1.427, 95% CI = 1.40−1.44). The rising trend of male injured hospitalized patients was also greater than that of female injured hospitalized patients. Conclusion: Males were more at risk of injury than females. Gender differences were related to the increased risk of epidemiological characteristics of injury.

مستخلص: Background: The range of options people have to do the things they value in life may have strong effects on their well-being. This is especially true for young adults, as their opportunities and choices may affect both their current and future lives. This study follows Sen's capability approach (CA) to assess young people's well-being in terms of capabilities and functionings.
Methods: Repeated cross-sectional data from the Young Adult Survey Switzerland were used for the analysis (N = 58,615). Descriptive statistics were applied to analyze the distribution of capabilities and functionings according to individuals' capital equipment. Finally, multivariate regression analyses were conducted to investigate associations between social, cultural, and economic capital; overall capabilities; and functionings.
Results: Young men with lower capital equipment rated their capabilities and functionings lower than others. Capabilities and corresponding functionings differed in the domains of health, happiness, and intellectual stimulation. Multivariate analysis confirmed the effects of social, economic, and cultural capital on both overall capabilities and functionings.
Conclusions: Young men differ in their perceived capabilities and functionings in different life domains according to their equipment with different forms of capital. To better understand the mechanisms underlying the transformation of capabilities into functionings, future studies should analyze issues of choice and adaptation to restricted living conditions.

مستخلص: Recreational road running is growing in popularity and has been linked to numerous mental and physical health benefits. However, we know little about what environmental preferences or concerns runners have regarding participation in the sport, and whether differences exist across age and gender. We conducted a cross-sectional survey on recreational road runners to investigate the type of built and natural environments road runners prefer, as well as the safety and health concerns that may affect runners' choice of environment. Responses were analyzed by age and gender. A total of 1228 road runners responded to the survey; 59.6% of respondents were women and 32.1% of respondents were men. Most respondents preferred to run on asphalt or sidewalk surfaces, and preferred well-lit, tree-lined routes. Major concerns for both men and women include animals and dangerous road conditions. Men and women differed significantly in their responses to the importance of running around others and their primary concerns while running. Results of this study serve to deepen our understanding of recreational road runners' environmental preferences and concerns, providing valuable information for public health officials and city planners alike. This information must be considered if we are to continue to encourage uptake of running as a sport and reap its health effects.

مستخلص: Background : Sedentary behavior has been considered an independent risk factor to health. The aim of this systematic review and meta-analysis was to examine associations between objectively measured sedentary time and physical fitness components in healthy adults. Methods : Four electronic databases (Web of Science, Scopus, Pubmed and Sport Discus) were searched (up to 20 September 2020) to retrieve studies on healthy adults which used observational, cohort and cross-sectional designs. Studies were included if sedentary time was measured objectively and examined associations with the health- or skill-related attributes of physical fitness (e.g., muscular strength, cardiorespiratory fitness, balance). After applying additional search criteria, 21 papers (11,101 participants) were selected from an initial pool of 5192 identified papers. Results : Significant negative associations were found between total sedentary time with cardiorespiratory fitness (r = -0.164, 95%CI: -0.240, -0.086, p < 0.001), muscular strength (r = -0.147, 95%CI: -0.266, -0.024, p = 0.020) and balance (r = -0.133, 95%CI: -0.255, -0.006, p = 0.040). Conclusions : The evidence found suggests that sedentary time can be associated with poor physical fitness in adults (i.e., muscular strength, cardiorespiratory fitness and balance), so strategies should be created to encourage behavioral changes.

مستخلص: Joining an online social support group may increase perceived membership to a community, but it does not guarantee that the community will be available when it is needed. This is especially relevant for adults with Chronic Obstructive Pulmonary Disease (COPD), many of whom reside in rural regions and continually negotiate their illness identity. Drawing from social support literature and communication theory of identity, this cross-sectional study explored how COPD illness and geographic identities interact to influence patients' perceived availability of online social support. In April 2018, 575 adults with a history of respiratory symptoms completed an online survey. Patients with a COPD diagnosis reported greater availability of online support. This was partially mediated by a positive degree of COPD illness identity (i.e., being diagnosed with COPD, a history of tobacco use, severe respiratory symptoms, high disease knowledge, and low income but high education). The relationship between COPD illness identity and the availability of online support was strongest among those with low rural identity; however, at lower levels of COPD illness identity, participants with high rural identity reported the greatest degree of available online support. Results have important implications for tailored education approaches across the COPD care continuum by illness and geographic identities.
