مستخلص: The Safe Home Care Project investigated both qualitatively and quantitatively a range of occupational safety and health hazards, as well as injury and illness prevention practices, among home care aides in Massachusetts. This article reports on a hazard identified by aides during the study's initial focus groups: smoking by home care clients on long-term oxygen therapy. Following the qualitative phase we conducted a cross-sectional survey among 1.249 aides and found that medical oxygen was present in 9 percent of aide visits (314 of aides' 3,484 recent client visits) and that 25 percent of clients on oxygen therapy were described as smokers. Based on our findings, the Board of Health in a local town conducted a pilot study to address fire hazards related to medical oxygen. Medical oxygen combined with smoking or other sources of ignition is a serious fire and explosion hazard that threatens not only workers who visit homes but also communities. [ABSTRACT FROM AUTHOR]

مستخلص: Approximately 20 million nonsmoking workers are employed in workplaces without restrictions on smoking and are potentially exposed to secondhand smoke—a Class A carcinogen. These workers are largely in the service industry, in southern and western states and in non-urban areas. The Occupational Safety and Health Administration's (OSHA's) 1994 proposal for smoke-free workplaces (withdrawn in 2001) was attacked by many interest groups that may no longer oppose this protection. Federal regulation for smoke-free workplaces is needed for workers, who are not protected by state and local smoke-free laws. This policy could save thousands of lives each year, and prevent significant illness. Twenty-one states have "state plans" that would allow more protective laws. Of the 29 states under OSHA, 11 have comprehensive smoke-free statutes. Changes in the policy environment and in institutions such as unions, restaurant associations, and the tobacco industry since 2001 may improve the prospects for federal action and reduce disparities that currently characterize exposure to secondhand smoke. مستخلص: The prevalence of smoking among Latino immigrant workers needs to be addressed with culturally and linguistically appropriate smoking cessation programs. One innovative approach to reaching these workers is through worker health and safety training programs. These programs offer multiple opportunities to provide smoking cessation messages, a "teachable moment" to focus on health issues, and the likelihood that peers and family members will share these messages. A collaborative effort between an occupational health and safety training facility and a university utilizing Latino and non-Latino personnel developed a smoking cessation program integrated with worker health and safety training. Preliminary results of an evaluation of the program indicate that the program was able to reach a wide range of immigrant workers and improve their knowledge of the risks of smoking and the effectiveness of different methods of quitting smoking.
