    مستخلص: Forming judgements about registrars in a workplace-based assessment context is not easy. Of course, the workplace is an important environment for learning, but it's also often very busy and sometimes complex. Assessment judgements can, therefore, not always be standardised and structured. Nevertheless, judgements have to be defensible and fair. In this paper we will discuss the main guidelines to ensure that assessment judgements are defensible and fair. We will approach these from a perspective of assessment for learning - with the intent to use feedback in order to guide learning - and from the perspective of assessment of learning, using judgements to determine whether a registrar is ready to progress to a next phase. Core elements of good judgement are transparency in the form of constructive and actionable feedback in dialogue with the registrar, and procedural fairness in the form of multiple judgements, over time and with clear documentation. This all needs to be supported by a training quality assurance system.

    مستخلص: The Medical Specialties council of the Royal Dutch medical association (CGS) has published an innovative perspective on the future landscape of specialisms in medicine. The proposal introduces generalism as the standard of training and professional practice, which represents a fundamental change from the current system. The envisioned new structure would consist of a number of broad core specialisms, emphasizing generalist knowledge and skills, and additional subspecialties for highly complex care. This is expected to enhance healthcare efficiency and improve patient treatment. The plan meets changing healthcare needs and accommodates the preference of young doctors regarding their residency training and careers.The CGS wants to further explore the new perspective with all parties involved.

    مستخلص: The prevalence of disorders related to the movement apparatus such as osteoarthritis and neck/back complaints is increasing, thereby compromising the accessibility and affordability of movement care. Besides, these complaints cause high burden of disease, high sick leave and decreased self-sustainability. These developments demand an integral multidisciplinary and line transcending approach. Within the field of movement care several initiatives are already developed such as the Beweeghuis Network, Network Osteoarthritis, One-and-a-half line outpatient clinic Zuyderland. If healthcare evaluation of these initiatives show positive effects, these examples of network medicine can give answer and substance to the challenges and assignments discussed in the Integral Care Act. An important condition for upscaling on a national level is a new funding model in which prevention of care is being rewarded as well.

    مستخلص: A young woman, known to have glycogen storage disease type 1B (GSD1B) presents with severe periodontitis. GDS1B causes decreased hepatic and renal glucose production and in many cases neutropenia and neutrophil dysfunction leading to recurrent infections. It was decided to treat the patient by extraction of the most affected teeth and retention of the remaining teeth through periodontal treatment, both with antibiotic prophylaxis. After a follow-up period of 1.5 years, during which there was no visible improvement, it was decided to do a full dental extraction and fabricate complete dentures. Due to subsequent bone resorption in both jaws, the dentures were not functional. After consulting the internist and the oral and maxillofacial surgeon, the decision was then made to place dental implants in both the upper and lower jaw for implant-supported prosthetics. After successful treatment and an osseointegration period, the prosthetics were placed. 1 year after placement, there is a stable implantological situation, without pockets or apparent bone loss. The start of SGLT2 medication may have played a significant role in this.

    مستخلص: Digital interdisciplinary consultation is an asynchronous form of consultation in which the general practitioner (GP) can consult a medical specialist. We provide an overview of the various platforms available in the Netherlands and we performed a literature review on the effect on referrals, patient satisfaction and satisfaction of the GP and medical specialist. Until October 2022, we traced nine different platforms that enable digital interdisciplinary consultation between the GP and medical specialist in the Netherlands. We identified nine randomized controlled trials (RCTs) and three systematic reviews until the COVID-19 period. The RCTs did not show a significant effect on our outcome measures. Observational studies on the other hand show a reduction of referrals by the use of digital interdisciplinary and have high satisfaction rates among GPs, medical specialists and patients because it is perceived as efficient and it improves access to specialized care for the patient.

    مستخلص: Rudolf Virchow regarded medical education that did not include the history of medicine as barbarism. Despite this view, current education on medical history in medical faculties in the Netherlands is minimal in terms of the number of teaching staff and the hours spent on the subject. Important arguments for teaching medical history to medical students are academic development, a better understanding of the historic fundamentals of research, clinical practice and the medical profession and, more recently, a better understanding of the historical and social context of medicine. Modern education on the history of medicine provides a perfect opportunity for critical reflection on the complexity of modern-day medicine. On the basis of these arguments, history of medicine belongs within the framework for Undergraduate Medical Education as an independent domain with appropriate assessment.

    مستخلص: Since the introduction of the Health Insurance Act, effectiveness has been one of the criteria for coverage decisions in the Netherlands. The Health Insurance Board, later succeeded by Zorginstituut Nederland, elaborated that criterion: when assessing whether care meets the "state of science and practice", it uses the principles of evidence-based medicine. In recent years, several organisations have called for the structural evaluation of health care interventions and the removal of non-effective care from the benefits package. This intention was also reflected in the Integral Care Agreement concluded last year.

    مستخلص: Pharmacogenetics holds the promise of personalized medicine, resulting in higher effectiveness and fewer adverse effects. Yet, the clinical benefit of a pre-emptive pharmacogenetic test has not been demonstrated rigorously. Recently an open-label real-world implementation study has been published, in which patients were randomized to either genotype-informed treatment (based on a 12-gene pharmacogenetic panel) or standard treatment. The study shows that genotype-informed prescription of different types of medication, i.e., opioids, anticoagulants and antidepressants, leads to a 30% reduction of clinically relevant adverse effects. This result is promising and indicates that genotype-informed treatment improves medication safety. Unfortunately, the influence of genotype-informed treatment on the balance between effectiveness and adverse effects could not be assessed and cost-effectiveness data are pending. Hence, a pharmacogenetic panel and a DNA medication pass for everyone are on the horizon, but not yet there.

    مستخلص: Scientific research into natural products has substantially contributed to our current drug armamentarium. This research has yielded numerous novel molecular structures and it has also increased our knowledge of pharmacological mechanisms of action. Moreover, ethnopharmacological studies have regularly shown that the traditional use of a natural product may correlate with the pharmacological action of its components and their derivatives. Nature still has much more in store for healthcare than a bunch of flowers for the bedridden patient. To secure that future generations can fully reap these benefits, it is essential to conserve the biodiversity of natural resources and the indigenous knowledge about their bioactivity.

    مستخلص: A working group of the 'Resident Education 2025' project of the Association Medical Specialists has the task of developing a training structure for future medical specialist, which is more in line with the desired interprofessional collaboration within a network, with the right care on the right place. Responding to the required flexibility and deployment of medical specialists, also outside the intramural environment. In doing so, we want to safeguard and strengthen the enthusiasm of future and current specialists, among other things through a better work-life balance. A sketch of a training structure has been made, in which the new colleagues first broadly orientate themselves in the various options. Subsequently, by choosing a specific path (of EPAs) which can lead into a medical specialty in a more generalist sense and possibly further differentiate into a sub-specialist. Digital education is offered nationally with various themes, which can be choices in terms of subject and time at which this can be followed. This concept will be further elaborated by the working group in consultation with the various stakeholders, while also considering all obstacles that could stand in the way of this change.The working group Training structure hopes to start a discussion about a possible new training structure for the medical specialist of the future.
