    الوصف: Summary: Background: Mucositis is a common and highly impactful side effect of conventional and emerging cancer therapy and thus the subject of intense investigation. Although common practice, mucositis assessment is heterogeneously adopted and poorly guided, impacting evidence synthesis and translation. The Multinational Association of Supportive Care in Cancer (MASCC) Mucositis Study Group (MSG) therefore aimed to establish expert recommendations for how existing mucositis assessment tools should be used, in clinical care and trials contexts, to improve the consistency of mucositis assessment. Methods: This study was conducted over two stages (January 2022–July 2023). The first phase involved a survey to MASCC-MSG members (January 2022–May 2022), capturing current practices, challenges and preferences. These then informed the second phase, in which a set of initial recommendations were prepared and refined using the Delphi method (February 2023–May 2023). Consensus was defined as agreement on a parameter by >80% of respondents. Findings: Seventy-two MASCC-MSG members completed the first phase of the study (37 females, 34 males, mainly oral care specialists). High variability was noted in the use of mucositis assessment tools, with a high reliance on clinician assessment compared to patient reported outcome measures (PROMs, 47% vs 3%, 37% used a combination). The World Health Organization (WHO) and Common Terminology Criteria for Adverse Events (CTCAE) scales were most commonly used to assess mucositis across multiple settings. Initial recommendations were reviewed by experienced MSG members and following two rounds of Delphi survey consensus was achieved in 91 of 100 recommendations. For example, in patients receiving chemotherapy, the recommended tool for clinician assessment in clinical practice is WHO for oral mucositis (89.5% consensus), and WHO or CTCAE for gastrointestinal mucositis (85.7% consensus). The recommended PROM in clinical trials is OMD/WQ for oral mucositis (93.3% consensus), and PRO-CTCAE for gastrointestinal mucositis (83.3% consensus). Interpretation: These new recommendations provide much needed guidance on mucositis assessment and may be applied in both clinical practice and research to streamline comparison and synthesis of global data sets, thus accelerating translation of new knowledge into clinical practice.     الوصف: Introduction: Oral squamous cell carcinoma, which is known as the most common cancer of the oral cavity, is associated with high morbidity and mortality rate. To date, many efforts have been made to find effective methods to stop the growth of cancer cells. One of the emerging methods in this field is the application of low-level lasers. Materials and Methods: Human head and neck carcinoma cell lines were prepared and irradiated with four different wavelengths of low-level laser with a density of 1 j/cm2 and a power of 100 mW. The expression of the Bcl-2 and Bax genes and the Bax/Bcl-2 ratio were investigated by real-time PCR. Results: The highest percentage of Bcl-2 gene expression was related to 660nm wavelength and the highest percentage of Bax gene expression was related to 810 nm infrared wavelength. The lowest level of expression of the Bcl-2 gene related to the 810nm infrared wavelength and the Bax gene related to the 532nm wavelength was obtained. The lowest Bax/Bcl-2 ratio was obtained at the wavelength of 660nm and the highest ratio was obtained at the wavelength of 810nm. Conclusion: Despite the extensive studies conducted in the field of low-level laser application in oncology, more studies are needed to investigate the effect of this technology on cancer cells.     الوصف: Abstract Background and Aims Oral mucositis is one of the most serious complications due to chemotherapy and radiotherapy in head and neck cancer treatment. Oral mucositis causes a wide range of clinical signs and symptoms, such as ulcers, pain, and dysphagia. Additionally, because of speech limitations, patients' self‐esteem will decrease, ultimately causing reduced quality of life. The primary objective of this study was to investigate the role of diet in the onset and progress of mucositis induced by chemotherapy and radiation therapy in patients with cancers. Methods In this study, 121 patients with a mean age of 51.43 ± 13.08 years were selected randomly and referred to the cancer institute, where they underwent their first phase of chemotherapy. In this step, patients were examined and their severity of oral mucositis was graded according to the World Health Organization criteria. They completed a 3‐day allergen food recall and dietary recommendations were met. After completing the forms, four questionnaires were filled out for each patient, the patient's nutrition was analyzed using the N4 software, and the amount of macro‐ and micronutrients was measured. Results Micronutrients such as aspartic acid, glycine, serine, proline, alanine, arginine, glutamic acid, and vitamin B12 and macronutrients such as rose water, sausage, beverages, coffee, and lamb meat were examined, and a significant difference was observed between groups (grade 1 and 2 mucositis) (p

    الوصف: Statement of the Problem: Oral squamous cell carcinoma (OSCC) constitutes more than 90% of oral malignancies. The main risk factors of OSCC include cigarette smoking and alcohol. However, since not all smokers or alcohol drinkers develop this disease, other factors have also been suggested including genetic characteristics of every person to be implicated in the probability of developing OSCC. Purpose: Our aim in this study is to investigate the possible relationship between fingerprint patterns and the probability of developing OSCC. Materials and Method: In a cross sectional study, we had 140 patients in 2 groups as OSCC and cancer free. Fingerprints were recorded by fingerprint scanner device. The fingerprint patterns were categorized into three major groups and four subgroups. Groups were tested by chi-square. Results: The relationship between the main fingerprint patterns and incidence of OSCC became significant (p= 0.037). The frequency of the main pattern of Arch was significantly higher in the experimental group than in the control group (p< 0.05). Considering the main patterns of Loop and Whorl, no significant difference existed between the two groups. Furthermore, the frequency of subtype patterns of Double Whorl and Central Pocket Whorl was significantly higher in the control group than in the experimental group (p< 0.05). Conclusion: Since dermatoglyphics is contingent upon genetic variations, fingerprint can be used for investigating the susceptibility of people in developing different diseases, though further studies are required in this regard. This method is in no way a substitute for gold standard methods for diagnosis.

    الوصف: Abstract Oral focal mucinosis (OFM) is an extremely rare benign lesion of the oral cavity with unknown etiology, considered the oral counterpart of cutaneous focal mucinosis. It occurs mainly in women in the fourth and fifth decades of life. It has no characteristic features, and diagnosis depends on histological evaluation. Its pathogenesis is related to the excessive production of hyaluronic acid by fibroblasts during collagen production, which leads to focal myxoid degeneration. To date, ten documented cases have been reported in the literature in adolescents. This paper reports a rare case of OFM with a narrative review of the available literature.

    الوصف: Background: This study compared the diagnostic efficacy of VistaCam iX infrared camera, visual inspection, and bitewing-radiographs for the detection of primary occlusal caries of permanent teeth. Materials and Methods: In this in vitro experimental study, 80 extracted human premolars were evaluated. The occlusal surfaces of these teeth were demineralized by immersion in a demineralizing agent. Then, the International Caries Detection and Assessment System (ICDAS II), bitewing-radiography, and Proxi head of VistaCam iX were used to inspect them. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated for each diagnostic modality. Data were analyzed using SPSS. Twenty-five at P < 0.05 level of significance with one-way analysis of variance and Games–Howell test. Results: Bitewing-radiography had significantly lower sensitivity than ICDAS II and VistaCam (P < 0.05). ICDAS II was comparable to VistaCam, with no significant difference in sensitivity (P > 0.05). ICDAS II had a significantly higher PPV than bitewing-radiography and VistaCam (P < 0.05). The sensitivity of bitewing radiography was significantly lower than that of ICDAS II and VistaCam (P < 0.05). ICDAS II was comparable to that of VistaCam with no significant differences in sensitivity (P > 0.05). ICDAS II had a considerably higher PPV than bitewing-radiography and VistaCam (P < 0.05). The NPV of ICDAS II visual inspection was significantly higher than that of bitewing-radiography and VistaCam (P < 0.05). The ICDASS II and VistaCam had a repeatability coefficient of 47.4%. For bitewing-radiography and VistaCam, this value was 44.2% and 83.4% for ICDAS II and bitewing-radiography. Conclusion: Visual inspection seems to be superior to bitewing-radiography and VistaCam in detecting primary occlusal caries of permanent teeth.

    الوصف: Background: Medication related osteonecrosis of the jaw (MRONJ) is a chronic condition of the oral cavity resulting in mucosal ulceration and exposure of underlying necrotic bone, and the ensuing secondary complications. The aim of this study was to evaluate the relationship between vitamin D deficiency and osteonecrosis of the jaw related to bisphosphonates. Materials and Methods: This was a case-control study. The samples were 20 patients taking bisphosphonates, that 10 of them were with MRONJ and 10 were non-MRONJ. Clinical examination of patients was performed to diagnose jaw osteonecrosis. Demographic data of the patients were recorded including age, sex, type of drug, duration and cause of drug intake, and measurement of serum vitamin D levels. Data were analyzed using SPSS software. Results: In the patients without MRONJ, the mean age was 60.60 (±14.975) years, and in the patients with MRONJ, the mean age was 68.30 (69.92) years. As a whole, of the 16 female patients in this study, 10 cases (62.5%) were non- MRONJ and 6 cases (37.5%) were suffered by MRONJ. All of the male patients presented with MRONJ. In control group mean of vitamin D was 63.990 ng/ ml (±29.796) and in case group mean of vitamin D was 29.510 ng/ml (±23.723). The serum level of vitamin D (25-OHD) was significantly higher in control group than in the case group (p=0.010). Conclusion: According to our result, there were statistically significant relationship between age, sex, type of drug, vitamin D level, and MRONJ (p>0.05).     الوصف: Introduction: Recurrent aphthous stomatitis (RAS) is a multifactorial recurrent oral lesion which is an autoimmune disease. TH1 cytokines are the most important etiological factors. Autoimmune thyroid disease (ATD) is one of the most common autoimmune diseases and generally coexists with other autoimmune diseases. This study assessed the prevalence of thyroid disease in patients with recurrent aphthous stomatitis. Materials and Methods: This case control study assessed 100 patients with RAS. Oral medicine specialists diagnosed RAS clinically; venous blood samples were analyzed for thyroid stimulating hormone (TSH), free triiodothyronine (fT3), total thyroxine (fT4), thyroglobulin, anti-thyroid peroxidase antibody (anti-TPO) and anti-thyroglobulin antibody (anti-TG) levels. Results: Fifty patients with RAS aged between 18-42 years (28.5±5.8) and 50 healthy volunteers aged 19-45 years (27.3±5.4) participated. In RAS patients, fT3 and TSH levels were significantly higher (P=0.031, P=0.706); however, fT4 level was lower in the RAS group (P=0.447). Anti TG and anti-TPO levels were significantly higher in the RAS group (P=0.008, P=0.067). Conclusion: Our study showed that ATD prevalence was significantly higher in RAS patients. Based on this study, we recommend assessment of thyroid hormones and antibodies in RAS patients.

    الوصف: Oral lichen planus (OLP) is a chronic autoimmune cell-mediated disease affecting mucocotaneous tissues of oral cavity. Recent studies revealed that psychological stress may act as a trigger to initiate clinical symptoms. An 8-year-old girl complaining of oral ulcer, pain and irritation in mouth with onset of 3 months ago, was admitted to oral medicine department. Her medical history was negative except for severe psychological trauma in last 6 months. Intraoral examination showed bluish purple atrophic erosive striations with hyperkeratosis. Pathological study of buccal lesions confirmed OLP diagnosis. As all primary investigations were negative; psychogenic origin was suspected. Although it is rare for a child to have OLP but it should be taken into consideration in differential diagnosis of red and white oral lesions in children. Huge impact of psychological factors in the occurrence of oral diseases have suggested that they should be treated in combination with psychotherapy.

    الوصف: Background and Aims: The most prevalent endocrine disease with unknown etiology among women in their fertility period is Polycystic Ovarian Syndrome (PCOS). The disease causes some changes in oxidative stress system, which in turn, is associated with such diseases as metabolic syndrome, diabetes, cardiovascular diseases, and periodontitis. The present study investigated periodontal status and total antioxidant status in serum of women with PCOS. Materials and Methods: In this cross-sectional, analytic study eighty women, 40 with PCOS as cases and 40 infertile women without PCOS participated. Interview, oral examination and radiographs, and laboratory tests served as data collection tools. Body Mass Index (BMI), Community Periodontal Index (CPI), Periodontal Disease Index (PDI), bone loss, and total anti-oxidant status (TAS) in serum of the participants were measured. Chi-squre test, Mann-Whithney test, and linear regression served for statistical analysis. Results: While in case group 21 patients had bone loss, in control group bone loss was found in 11 pateints (P=0.022). The distribution of maximum CPI score among cases was significantly different from that among controls in that more frequent higher scores existed among cases (P=0.016). The mean PDI in case and control group was 6.23±3.3 and 4.48±2.6, respectively (P=0.015). In general linear model the level of serum TAS was significantly associated with CPI (P=0.039) and BMI (P=0.019). Conclusion: Women with PCOS seem to be more susceptible to periodontal disease compared to other women. This calls for comprehensive periodontal care and regular dental visits for patient with PCOS.
