    الوصف: IntroductionThe present study conducted a secondary data analysis of a comprehensive survey from Child Guidance Centers in Japan to identify factors that are associated with child abuse severity in infancy (0–3 years, 1,868 cases) and preschool age (4–6 years, 1,529 cases). A predictive model for abuse severity was developed.MethodsThe data originated from a nationwide survey that was conducted in April 2013, consisting of details of abuse cases, including child characteristics, abuser attributes, and family situation. Abuse severity was assessed on a five-level scale (suspected, mild, moderate, severe, and life-threatening) that was converted into a binary outcome. Logistic regression analysis was used to create a predictive model using two-thirds of the data, which was validated with the remaining third of the data.Results and discussionAs a result, in infancy, risks of severity increased with younger age of the abused child, physical abuse, neglect, witnessing domestic violence, and the involvement of Child Guidance Centers or hospitals in detection. The abuser's mental problems and cumulative child damage contributed to severity. For preschool age, similar factors applied, with additional risks that included abuse overlap and guardian separation. Cumulative abuser issues and child physical damage impacted severity. Validation yielded moderate prediction accuracy (areas under the curve: 0.703 and 0.714).

    الوصف: Four long-spined sea urchin species in the genus Diadema are known to occur around the Japanese Archipelago. Three species (D. savignyi, D. setosum, and D. paucispinum) are widely distributed in the Indo-Pacific Ocean. The fourth species was detected by DNA analysis among samples originally collected as D. savignyi or D. setosum in Japan and the Marshall Islands and tentatively designated as Diadema-sp, remaining an undescribed species. We analyzed nucleotide sequences of the cytochrome oxidase I (COI) gene in the "D. savignyi-like" samples, and found all 17 individuals collected in the mainland of Japan (Sagami Bay and Kyushu) to be Diadema-sp, but all nine in the Ryukyu Archipelago (Okinawa and Ishigaki Islands) to be D. savignyi, with large nucleotide sequence difference between them (11.0%±1.7 SE). Diadema-sp and D. savignyi shared Y-shaped blue lines of iridophores along the interambulacrals, but individuals of Diadema-sp typically exhibited a conspicuous white streak at the fork of the Y-shaped blue iridophore lines, while this feature was absent in D. savignyi. Also, the central axis of the Y-shaped blue lines of iridophores was approximately twice as long as the V-component in D. savignyi whereas it was of similar length in Diadema-sp. Two parallel lines were observed to constitute the central axis of the Y-shaped blue lines in both species, but these were considerably narrower in Diadema-sp. Despite marked morphological and genetic differences, it appears that Diadema-sp has been mis-identified as D. savignyi for more than half a century.

    الوصف: Ovarian carcinosarcoma composed of high-grade carcinoma and sarcoma is an extremely rare neoplasm and typically occurs in postmenopausal women aged over 60 years. A 73-year-old female, gravida three para three, presented to our hospital with right lower abdominal pain. Right pelvic solid tumor with ascites was detected on pelvic ultrasound examination. She underwent hysterectomy, bilateral salpingo-oophorectomy and partial omentectomy, but the tumor had invaded to the right ureter, and some fragile tumor could not be taken (sub-optimal surgery). On the imprint and ascitic cytology specimens during operation, atypical cells suggestive of adenocarcinoma and spindle atypical cells with immunocytochemically vimentin positive were found. The resected tumor was histopathologically carcinosarcoma consisted of serous adenocarcinoma, chondrosarcoma and fibrosarcoma. Immunohistochemical analysis revealed that adenocarcinoma cells were positive for AE1/AE3 and fibrosarcoma cells stained with vimentin. The final diagnosis was the right ovarian carcinosarcoma (stage pT3CNxMx). Microsatellite instability was stable and BRCA1/2 mutations could not be found in the carcinosarcoma cells. The patient was given four cycles of chemotherapy with paclitaxel, carboplatin and bevacizumab regimen, and thereafter she was treated with the ifosfamide and cisplatin because of slight elevation of serum CA125.

    الوصف: Endometrial neuroendocrine tumors are rare, accounting less than 1% of endometrial cancers. They include small cell neuroendocrine carcinoma (NEC) and large cell NEC and usually occur in postmenopausal patients. Although common symptoms include postmenopausal bleeding, most patients are diagnosed at an advanced stage. We report an extremely rare case of small cell NEC developed in the endometrium of a 75-year-old Japanese woman. This case was initially suspected based on the findings of endometrial cytology: a number of small round malignant cells were present on the endometrial smear specimens. The immunocytochemical and immunohistochemical examinations revealed diffusely cytoplasmic positive reaction of neuron specific enolase (NSE) and partially membranous positive reaction of CD56 in the small cancer cells. They showed PAX-8-positive reaction, but did not express microsatellite instability high. Electron micrography showed several dense-core secretory granules in the cytoplasm of cancer cells. Despite multiple lung metastases, the patient underwent a hysterectomy and salpingo-oophorectomy in order to control excessive genital bleeding. She received six courses of adjuvant chemotherapy based on etoposide and cisplatin, and survived healthy eight months after the first visit without any viability of lung metastases.
