مستخلص: Competing Interests: Disclosure All authors disclosed no financial relationships. Commentary This case illustrates a patient with the extremely rare McKittrick-Wheelock syndrome. Readers can be forgiven for never having heard of this syndrome. The syndrome involves large villous adenomas that secrete mucin rich in electrolytes that can lead to prerenal kidney injury as well as dehydration from secretory diarrhea. The fact that this could all be caused by a polyp is counterintuitive but nonetheless true. The mainstay of therapy is to remove the offending adenoma, either by endoscopy or surgery, as was performed here. The lesions that cause this syndrome can be very large, as was the case in this patient. Physicians unfamiliar with the syndrome may not make the connection between the patient’s diarrhea and kidney injury and the adenoma. Still, prompt recognition of this constellation of findings can produce an excellent outcome and avoid significant adverse events for the patient. Douglas G.
