مستخلص: Brief Background Cone beam computed tomography (CBCT) has emerged as a three-dimensional imaging technology which finds its application in all branches of dentistry. Its advantages being accessibility, easy handling, and it offers a real-size data set with multiplanar cross-sectional and 3D reconstruction based on a single scan with low radiation dose. We present a case of infra-temporal and temporal space infection caused due to backtracking of infection from the periapical area of 38 into the ramus superiorly and posteriorly, ultimately causing a lingual cortex perforation and spreading the infection superiorly to the temporal region. The CBCT aided us in reaching a diagnosis, studying the unusual intra-osseous course taken by the infection and then guided us to take an extra-oral approach for therapeutic incision and drainage.
