This article discusses a case of gastric and duodenal pseudomelanosis in a 67-year-old man with a medical history of diabetes, hypertension, ischemic heart disease, and chronic kidney disease. The patient had normochromic normocytic anemia and underwent upper GI endoscopy and colonoscopy, which revealed dark-brown pigmented mucosa in the stomach and duodenum. Biopsies showed dark-brown and black pigments in macrophages, and the pigmentation resolved after hemodialysis and cessation of certain medications. Pseudomelanosis is a rare and benign finding in the upper GI tract, and its exact cause is unknown, although risk factors include chronic oral iron therapy and certain medications.
