مستخلص: This article discusses the challenges of treating elderly patients with acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS) and the poor prognosis associated with these conditions. It highlights the use of hypomethylating agents (HMAs) as a potential treatment option, but notes that data for patients with hypocellular bone marrow are lacking. The study evaluated the efficacy of low-dose melphalan on the clinical outcomes of patients with hypo-/normocellular MDS-EB and AML and found that some patients had a positive response to the treatment. The study also identified DDX41 variants as potential predictors of response to melphalan. Overall, the study suggests that low-dose melphalan may be an effective treatment option for patients with DDX41 variants and hypo-/normocellular AML and MDS-EB. [Extracted from the article]
