مستخلص: This article discusses the challenges of accurately identifying and staging gastric intestinal metaplasia (GIM) during endoscopy, particularly in countries with a low incidence of gastric cancer (GC). The current gold standard for GIM staging is random biopsy histopathology, but this method is poorly reproducible and may underestimate GIM. The study evaluates the clinical yield of an endoscopy-led risk stratification strategy using virtual chromoendoscopy (VCE) combined with targeted biopsies in low GC incidence settings. The results show that VCE with targeted biopsies has a diagnostic accuracy of 80.9% for GIM and can increase the detection rate of GIM in patients with premalignant gastric lesions. The findings support the use of endoscopy-led risk stratification in low incidence countries. [Extracted from the article]
