مستخلص: Tumors arising from the ventral pancreas tend to spread toward the superior mesenteric artery (SMA),[1] and consequently, the SMA is the artery most commonly involved by local extension of cancers of the pancreatic head and uncinated process. Most investigations of margin status after PD indicate particular prognostic significance for the status of the posteromedial resection margin surrounding the SMA.[2], [3] An artery-first approach permits early determination of resectability and facilitates en bloc resection of the pancreatic head nerve plexus near the SMA during PD. Here we present our artery-first technique, which can facilitate en bloc resection, confirm margin status of the retropancreatic nerve plexus (RNP) early during surgery, and minimize intraoperative bleeding.
