Cerebral perfusion is impaired for various reasons in coronary artery bypass surgery (CAB) operations. Coexistence of carotid stenosis further disrupts perfusion. In this case presentation, we aimed to present our anesthetic management during combined CAB and right carotid endarterectomy (CEA) surgery in a patient with a completely occluded right carotid artery and a 50-70% stenosis in the left carotid artery. Koroner arter baypas cerrahisi (KAB) ameliyatlarında serebral perfüzyon çeşitli nedenlerle bozulmaktadır. Buna ilaveten karotis darlığının olması perfüzyonu daha da bozar. Bu olgu sunumunda, sağ karotis arteri tamamen tıkalı olan ve sol karotis arterinde de %50-70 darlığı olan bir hastada kombine KAB ve sağ karotid endarterektomi (KEA) cerrahisi sırasındaki yönetimimizi sunmayı amaçladık.
