مستخلص: Introduction: Lymphomas with neoplastic transformation of lymphoid cells have variable mortality rates according to their subtypes, with a variable prognosis depending on the histological type and time of evolution, and may be reserved when there is no early and adequate intervention. Nasosinusal lymphomas are rare, with an insidious course, little symptomatic and with late diagnosis. Goals: To report a rare presentation of a primary Non-Hodgkin's Lymphoma of large B-cells of the maxillary sinus. Summary report: Male C.L, 74 years old, sought care for headache on the left side of the face with insidious onset and gradual worsening, associated with purulent rhinorrhea and epistaxis. Nasal endoscopy was performed, visualizing purulent secretion in the left middle meatus and partial edema due to a polypoid lesion extending to the nasal cavity. Non-contrast tomography showed material with soft tissue density, fully enlarging the left maxillary bone, extension to the nasal cavity, elongation of the ostiomeatal complex and subperiosteal maxillary reaction. Scheduled excisional biopsy of the lesion with left maxillary antrostomy via Caldwell-Luc. Histopathology showed immunohistochemical result of "Large B Cell Non-Hodgkin Lymphoma". The patient underwent specific chemotherapy and after 1 year of follow-up, there was no recurrence of the lesion. Conclusion: This presentation in a common place made us highlight the importance of endoscopic and tomographic studies for surgical planning, as part of a differential diagnosis of diseases that elongate the ostiomeatal complex.
