From the distal metacarpus/metatarsus, both tendons are enclosed in a communal tendon sheath held in close contact with the flexor aspect of the first phalanx by the middle annular ligament. The thin tendon sheath is incised, and with careful sharp dissection using a scalpel blade, the tendon is freed of connective tissue enabling the curved mosquito forceps to be forced under the tendon (Figure 10). A tenotomy/tendonectomy of both SDF and DDF tendons at: metacarpus/metatarsus the level of P1 when scarring from previous surgery at the metacarpus/tarsus makes tendon identification difficult.
