مستخلص: We investigated how the prescriptions of Kampo extract preparations prescribed over a period of 17 years and analyzed the trends and prescriptions by disease at the Saitama Medical Center attached to Jichi Medical University, which does not have the department of Kampo medicine as a specialized medical treatment. Based on these results, we considered the role of the pharmacist in the field of Kampo medicine. A total of 120,336 Kampo extract preparations were prescribed in 17 years, and the annual prescriptions increased year by year. Daikencyuto was the most prescribed extract preparations, followed by Goreisan which increased markedly from around 2009. An analysis of prescriptions in 2018 revealed that 91 types of extract preparations were prescribed, with 65.8% outpatient and 34.2% inpatient prescriptions. The department of Surgery prescribed these extract preparations more than any other. The distribution of prescribed patients according to gender was 41.2% male and 58.8% female. Kampo extract preparations were frequently prescribed to elderly patients in their 60s and 70s and female patients in their 40s and 50s, and two peaks were observed. The current state of Kampo prescriptions at our hospital showed a strong tendency to select Kampo extracts as treatment options of synthetic drugs, based on Western medical diagnostic criteria. Patients prescribed Kampo medicine should originally be diagnosed and prescribed based on classical diagnostic methods and medical evidence in modem medicine. Accordingly, attention should be paid to evidence and clinical practice guidelines in order to understand the prescription intentions of doctors other than Kampo specialists. In fact, various side effects such as mesenteric phlebosclerosis and pseudoaldosteronism and aggravation of diseases due to "mistreatment" have been observed occasionally in long-term use of Kampo medicines. Therefore, it can be said that experience and knowledge of the basics of classical Kampo diagnosis and clinical application are essential for the effective and safe use of Kampo medicines, regardless of whether providing the service on Kampo specialty outpatient or not. In such a situation, it is important for hospital pharmacists as pharmaceutical professionals to acquire qualifications actively as certified pharmacists of Kampo medicines and Chinese medicine doctors, and follow up doctors and patients by incorporating knowledge about Kampo prescriptions and analysis of diagnostic methods.
