Basal cell carcinoma is one of the most common malignancies located in the auricle. It is an aggressive tumor due to the local evolution, being able to invade adjacent tissues, but with a low rate of metastasis. Establishing a correct histological diagnosis is essential for proper therapeutic management and for the patient’s prognosis. The authors present the clinical case of a 74-year-old patient admitted to our clinic for a tumor of the left auricle. The problems encountered in establishing a correct histological diagnosis, the particularities of the complete surgical ablation of the tumor and the aspects related to the postresection auricle remodelling technique are presented, exemplified and commented, considering the practice of a surgical intervention in an area with profound esthetic implications. The short-term and medium-term results highlight the lack of tumor residue, the successful coverage of the postablative skin defect and a satisfactory local appearance in the left ear and face in general. The diagnostic and therapeutic management of ear malignancies involves a number of difficulties and must be established through interdisciplinary collaboration within a specialized oncology commission. Complete ablation of the tumor and obtaining safe negative margin, together with the reconstructive technique used after resection allowed good results in the short and medium term from an oncological, functional and esthetic point of view. Carcinomul bazocelular este una dintre cele mai frecvente afecțiuni maligne localizate la nivelul pavilionului auricular. Este o tumoră agresivă din cauza potențialului evolutiv local, putând invada țesuturile adiacente, dar prezentând în același timp o rată scăzută de metastazare. Stabilirea unui diagnostic corect din punct de vedere anatomopatologic este esențială pentru un management terapeutic adecvat și pentru prognosticul pacientului. Autorii prezintă cazul clinic al unui pacient în vârstă de 74 de ani, internat în clinica noastră pentru prezența unei formațiuni tumorale auriculare stângi. Sunt prezentate, exemplificate și comentate problemele întâmpinate în stabilirea unui diagnostic histopatologic corect, particularitățile intervenției chirurgicale efectuate, de ablație completă tumorală și aspecte legate de tehnica de plastie a pavilionului auricular postrezecție, având în vedere practicarea unei intervenții chirurgicale într-o zonă cu implicații estetice importante. Rezultatele pe termen scurt și mediu evidențiază absența restanței tumorale, acoperirea cu succes a defectului cutanat postablativ și un aspect local satisfăcător atât la nivelul urechii, cât și la nivel facial în general. Conduita diagnostică și managementul terapeutic al tumorilor maligne ale urechii implică o serie de dificultăți și trebuie stabilite prin colaborare interdisciplinară în cadrul unei comisii oncologice. Ablația completă a tumorii și obținerea marginilor negative de siguranță, împreună cu tehnica reconstructivă utilizată după rezecție, au condus la rezultate bune pe termen scurt și mediu din punct de vedere atât oncologic, cât și funcțional și estetic. Tratamentul chirurgical a fost singurul mijloc terapeutic utilizat în acest caz. [ABSTRACT FROM AUTHOR] |
