| Abstract (Chinese): | 该文报道1例妊娠中期瘢痕子宫伴不全纵隔子宫行依沙吖啶引产时不全子宫破裂患者的保守 治疗过程。该患者为2次剖宫产史合并纵隔子宫, 因胎儿发育畸形于孕中期引产, 引产方案为口服米非司酮+ 羊膜腔注射依沙吖啶。引产过程中患者因宫缩痛给予硬膜外分娩镇痛, 1 h后诉右侧下腹部压痛, 阴道出血60 mL, 色鲜红, 急查床边彩色多普勒超声(彩超), 考虑子宫破裂。内诊查宫口已近开全, 因胎儿孕周21周, 在宫 颈内口可触及胎儿肢体, 在充分开放静脉通路的同时给予钳取死胎(胎重700 g), 产后产妇生命体征平稳, 24 h 阴道出血量350 mL, 严密监测产妇生命体征、腹痛及阴道出血情况, 结合实验室检查及彩超结果给予抗炎、促 子宫收缩治疗, 6 d后产妇子宫复旧可, 少量恶露排出, 给予办理出院;47 d后门诊复查彩超子宫破裂长度较前 明显变小。 [ABSTRACT FROM AUTHOR] |
