مستخلص: Carpal tunnel syndrome being the most common entrapment neuropathy and can be managed with hydrodissection of the median nerve when conservative treatment options fail in mild-moderate disease cases. The principle behind hydrodissection is to free up the nerve from entrapment by separating the nerve from surrounding tissue. Fenestration of the flexor retinaculum can also help in the release of mechanical compression of the entrapped nerve, but poorly described in the literature and not routinely used. Here we present a case of a 72-year-old female patient with carpal tunnel syndrome managed with ultrasound-guided fenestration of the flexor retinaculum and hydrodissection of the median nerve with significant symptomatic and functional improvement over 2-month follow-up.
