The article focuses on the different types of errors in the implementation of a standardized IV insulin infusion protocol (IIIP) in the cardiovascular intensive care unit (CVICU). The purpose of the modification of IIIP from a published protocol based on the formula [(Current BG-60) X Sensitivity Factor] was to determine the insulin infusion rate. Five major types of errors that were identified with IIIP include failure to increase S, failure to decrease S and violation of protocol. |
