مستخلص: The intensive care unit (ICU) physical therapist must consider the safety of mobilizing patients, status post (s/p) acute ischémie stroke, that have been treated with tissue plasminogen activator (tPA). Tissue plasminogen activator is an encyme that initiates local fibrinolysis of a blood clot. Due to this fibrinolysis, the possibility of further bleeding is a concern when performing bed mobility, sitting and gait activities. In our facility, patients s/p ischémie stroke are not to be mobilized within 24 hours of administration of tPA. This has been an unwritten protocol based from the American Heart Association (AHA)/American Stroke Association (ASA) Guidelines for Early Management of Patients with Ischémie Stroke. Recently, neurology residents are requesting mobilization before 24 hours. We decided to use an evidence-based approach to form a clinical decision regarding the safety of mobilizing patients with an ischémie stroke s/p use of tPA. To illustrate the decision making process, a generic case of a typical patient seen in the intensive care unit was used. [ABSTRACT FROM AUTHOR]
