مستخلص: Objective: To identify factors associated with increased risk of contracting pulmonary tuberculosis (PTB) in adult HIV patients.Design: Case control study of tuberculosis (TB) cases registered in Hospital Kota Bharu, Kelantan, Malaysia within 2003 to 2004.Materials and Methods: Cases were 97 adult PTB patients with HIV-infection and controls were 97 adult PTB patients without HIV infection. Interviewer guided questionnaire was administered to determine the socio demographic data, personal history and medical history of patients while clinical and laboratory information were reviewed from their medical records.Results: In multiple logistic regression analysis, significant risk factors detected were history of TB contact (adjusted OR =4.94, 95%CI 1.73, 14.07), past history of TB (adjusted OR =4.59, 95%CI 1.06, 19.78) and chronic diseases (adjusted OR =4.41, 95%CI 1.28, 15.20). Being females (adjusted OR =0.06, 95%CI 0.02, 0.24) and married (adjusted OR =0.06, 95%CI 0.01, 0.25) or widowed (adjusted OR =0.14, 95%CI 0.02, 0.84) had protective effect towards the risk to contract PTB compared to those being males and singles respectively. Other significant risk factors were CD4 (adjusted OR =0.996, 95%CI 0.993, 0.999), TWBC (adjusted OR =0.76, 95% CI 0.62, 0.92) and ESR (adjusted OR =1.04, 95%CI 1.03, 1.06).Conclusion: This study showed that patients at high risk can be identified on the basis of demographic and clinical features which can be of great help in understanding about TB and HIV co-infection.
