Introduction: : Correction of Class II malocclusion with mandibular deficiency in growing patients requires an orthopedic approach associated with orthodontic treatment to, in addition to correcting tooth relationships, stimulate mandibular growth and reduce facial convexity. Mandibular propulsion can be used coupled to fixed orthodontic appliances, to carry out the orthopedic correction, together with the necessary orthodontic movements. Objective: This article presents a clinical case report, with a treatment protocol to correct this type of dentofacial deformity, associating mandibular propulsion to passive self-ligating brackets with individualized prescriptions. The aim of orthodontic treatment was to prepare the arches for orthopedic advancement, as well as to inhibit unwanted effects resulting from the forces generated by the mandibular propulsion. This approach promoted a clockwise rotation of 2.5° in the occlusal plane (FHP.OclPl from 2.5° to 5.0°); however, the mandibular plane was maintained (FMA=17°). The sagittal relationship between maxilla and mandible improved, with a 4° ANB reduction (ANB from 6° to 2°). Maxillary incisors increased inclination, promoting a horizontal trespass (overjet), allowing orthopedic correction with mandibular advancement. The inclination of the maxillary incisors increased by 19° (1.PP from 100° to 119°), and the lower incisors had an increase of 7° (IMPA from 93° to 100°). Facial convexity improved by 4° (FCA 163° to 167°). Conclusion: The association of the mandibular propulsion to passive self-ligating appliances with individualized prescriptions proved to be effective in correcting Class II malocclusion, promoting a good orthopedic effect and inhibiting unwanted orthodontic effects. INTRODUÇÃO: A correção da má oclusão de Classe II com deficiência mandibular em pacientes em fase de crescimento necessita de uma abordagem ortopédica associada ao tratamento ortodôntico para, além de corrigir as relações dentárias, estimular o crescimento da mandíbula e reduzir a convexidade facial. Os propulsores mandibulares podem ser utilizados acoplados aos aparelhos ortodônticos fixos, com o objetivo de efetuar a correção ortopédica juntamente com as movimentações ortodônticas necessárias. OBJETIVO: O presente artigo apresenta um relato de caso clínico, com um protocolo de tratamento para corrigir esse tipo de deformidade dentofacial, associando propulsores mandibulares aos braquetes autoligáveis passivos com prescrições individualizadas. O tratamento ortodôntico visou preparar as arcadas para efetuar o avanço ortopédico, bem como inibir os efeitos colaterais indesejados decorrentes das forças geradas pelo propulsor mandibular. Essa abordagem promoveu uma rotação horária de 2,5° do plano oclusal (PHF.POcl de 2,5° para 5,0°); porém, manteve o plano mandibular (FMA=17°). A relação sagital entre maxila e mandíbula melhorou, com uma redução de 4° no ANB (de 6° para 2°). A inclinação dos incisivos superiores aumentou, promovendo um trespasse horizontal, permitindo a correção ortopédica com o avanço mandibular. A inclinação dos incisivos superiores aumentou 19° (1.PP de 100° para 119°), e a dos inferiores aumentou 7° (IMPA de 93° para 100°). A convexidade facial melhorou 4° (Â Conv.Face de 163° para 167°). CONCLUSÃO: A associação do propulsor mandibular com os aparelhos autoligáveis passivos com prescrições individualizadas se mostrou eficiente na correção da má oclusão de Classe II, promovendo um bom efeito ortopédico e inibindo os efeitos ortodônticos indesejáveis.
