| الوصف: | Introduction : La varicocèle est une dilatation anormale des veines du plexus pampiniforme. Elle peut entrainer un dysfonctionnement et une atrophie testiculaire responsables d'anomalies du spermogramme. Matériel et méthodes : Nous avons mené une étude descriptive et analytique pendant la période de 2010 à 2018 dans le Service d'Urologie-Andrologie de l'Hôpital Central de Yaoundé. Etaient inclus tous les patients ayant bénéficié d'au moins un spermogramme et d'un dosage des hormones étudiées en pré- et postopératoire. Résultats : Au total 71 patients ont été enrôlés dans l’étude. L'âge moyen des patients était de 37,6 ±7,4. Le symptôme le plus rencontré était la douleur inguino-scrotale chez 85,90% des patients, la varicocèle était gauche dans 73,20% des cas et majoritairement de grade 3 dans 43,70% des cas. Le spermogramme préopératoire retrouvait une oligo-asthéno-tératospermie chez 42,3% des patients. Tous les patients ont bénéficié d'une cure chirurgicale par voie sub inguinale. Avant la chirurgie, la testostéronémie était basse chez 31% des malades et l’hormone folliculaire stimulante élevée dans 88,70% des cas. Après un suivi minimal de 3 mois, il y’avait une amélioration notable statistiquement significative des hormones sexuelles étudiées et de la qualité du sperme. Conclusion: La cure chirurgicale améliore de façon significative la fertilité des patients hypofertiles. English title: Variations in spermogram after varicocele repair in a tertiary hospital in Yaounde Introduction: Varicocele is defined as abnormal dilatation and enlargement of the venous scrotal pampiniform plexus. It can be responsible for abnormalities in the spermogram. Materials and methods. From 2010 to 2018 we retrospectively reviewed 71 patients in the Urology department of the Yaoundé Central Hospital. All patients performed at least one spermogram and hormone assessment pre- and postoperatively. Results. The average age of patients was 37.6± 7.4 years with extremes of 25 and 58 years. The most common symptom was inguinoscrotal pain in 85.90% patients. Varicocele was left side in 73.20% of cases and mostly grade 3 in 43.70% of the study population. The preoperative spermogram was normal in 15.5% of cases. oligoasthenoteratospermia was found in 42.3% of cases. All patients underwent surgery through a subinguinal approach. Before surgery, testosterone level was low in 31% of cases; Follicle-Stimulating Hormone level was high in 88.7% of patients. After a minimum follow up of 3 months, features of spermogram, testosterone and Follicle-Stimulating Hormone have improved. Conclusion. Surgery of varicocele significantly improves fertility. |
