الوصف: | Mestrado em Psicologia da Saúde e Reabilitação Neuropsicológica O presente trabalho teve como objetivo principal investigar, em idosos, a perceção da gravidade da insónia, assim como a associação do subtipo de insónia (inicial, intermédia ou final) com o cronótipo (matutinidade-vespertinidade), comparando participantes em regime de Estrutura Residencial para Pessoas Idosas (ERPI) e em regime de Centro de Dia (CD). Secundariamente, procurou-se explorar em idosos, o comportamento psicométrico das escalas ISI (Índice de Gravidade de Insónia, adapt. portuguesa de Clemente, 2007) e QCM (Questionário Compósito de Matutinidade, adapt. portuguesa de Silva et al, 1994). A ISI avalia a tipologia e a gravidade das insónias enquanto que o QCM avalia uma tendência em termos de matutinidade-vespertinidade. Foram ainda examinadas as associações das variáveis de sono e do cronótipo com a sintomatologia depressiva e o declínio cognitivo. Para o efeito usaram-se, respetivamente, a GDS-15 (Escala Geriátrica da Depressão – Versão reduzida de 15 itens, adapt. portuguesa: Apóstolo, 2011) e o MoCA (Montreal Cognitive Asessment, adapt. portuguesa: Freitas et al., 2013). A amostra compreendeu 60 idosos, 50% de cada sexo, 30 em regime de ERPI e outros 30 em regime de CD, entre os 60 e os 95 anos de idade, M=79.86 ± 9.04 (significativamente superior no grupo ERPI, M=86.17±6.37, do que no grupo CD, M=73.57±6.51). Ao nível da consistência interna das escalas, encontraram-se alfas de Cronbach α=0.768 para a ISI e α=0.797 para o QCM. Comparado idosos em regime de ERPI ou em regime de CD, o grupo em ERPI relatou intensidade de insónias significativamente superior, bem como comprometimento cognitivo ligeiramente superior e maior sintomatologia depressiva. Constatou-se ainda uma associação entre declínio cognitivo e sintomatologia depressiva, e quanto mais elevados foram estes dois índices, mais graves foram queixas de insónia. Em último lugar, comparando matutinos e vespertinos, as queixas de insónia “terminal” foram mais visíveis nos primeiros e as de insónia “inicial” nos segundos (embora tanto nuns como noutros, a insónia de manutenção também assuma relevância). Em conclusão, os resultados indicam-nos que a sintomatologia depressiva e o declínio cognitivo estão associados a uma pioria na qualidade de sono. O momento em que ocorre a insónia parece diferir conforme o tipo diurno, com as queixas de insónia inicial a serem mais intensas em vespertinos e as de insónia terminal mais intensas em matutinos. As comparações entre os grupos ERPI e CD revelaram muitas semelhanças, destacando-se, no entanto, a tendência para o grupo ERPI apresentar maior gravidade de queixas de insónias, o que pode dever-se a uma idade média superior. As escalas ISI e QCM, nas versões portuguesas, apresentaram adequadas qualidades psicométricas nesta amostra de idosos, embora pareça ser necessário adaptar 2 itens do QCM para a aplicação em idosos. The present study aimed to investigate the perception of insomnia severity and its association with chronotype (morningness-eveningness), comparing a sample of elderly in Residential Structure for Older People (ERPI) or in Daytime Centre for Older People (CD). Secondarily, we also sought to explore in the elderly the psychometric behaviour of the scales ISI (Insomnia Severity Index, Portuguese adapt. by Clemente, 2007) and QCM (Composite Questionnaire Morningness, Portuguese adapt. by Silva et al, 1994). ISI evaluates the type and severity of insomnia while QCM evaluates the participant’s tendency towards morningness-eveningness. Our study also sought to examine the associations between insomnia or chronotype, and depressive symptoms or cognitive decline. Therefore, we used the GDS-15 (Geriatric Scale Depression – Reduced version of 15 items, Portuguese adapt. by Apóstolo, 2011) and the MoCA (Montreal Cognitive Assessment, Portuguese adapt. by Freitas et al, 2013), respectively. The final sample included 60 elderly, 30 (50%) of each sex and regime (ERPI or CD), aged 60 to 95 years-old, M= 79.86 ± 9.04 (significantly higher in ERPI group, M= 86.17±6.37, when compared to the CD group, M=73.57±6.51). As to internal consistency of the ISI and the QCM, Cronbach coefficients were α = 0.768 and α = 0.797. Compared the elderly in ERPI vs. CD regimes, the ERPI group reported significantly more severe insomnia, showed slightly higher cognitive impairment and higher depressive symptoms. It was also found that the cognitive decline and depression symptoms were associated with each other, and the higher were these two indexes, the higher was the severity of insomnia. Finally it was found that early morning awakenings were more intense in morning-oriented participants, while sleep-onset difficulties were more severe in eveningness-oriented individuals (albeit both may also report relevant maintenance insomnia symptoms). In conclusion, our results indicate that depressive symptoms and the cognitive decline are associated with poorest sleep / higher insomnia complaints. Some types of insomnia in the elderly seem to be related to morningness-eveningness, so that sleep-onset insomnia seem more common in evening-oriented individuals and terminal insomnia in morning-type ones. The groups in ERPI or in CD seem to be essentially similar in the variables studied, excepted that the ERPI elderly appear to suffer more intense insomnia complaints, which might be due to their significantly older age. Both ISI and QCM show satisfactory psychometric properties, but our preliminary results suggest the need to adapt 2 QCM items to be used in older participants. |
