In this chapter, we review extended resuscitation considerations for patients who have suffered from a blast injury. The main focus of the body of literature on blast injury concerns the early management of these patients. While this information is valuable, it is also important to understand how to continue critical care for these patients after the initiation of resuscitation. Knowledge of the mechanism of the blast injury, and whether the patient sustained the injury in an enclosed space, informs the clinician of what clinical challenges to anticipate. Extended resuscitation issues include impact of resuscitation on the evolution of central nervous system, pulmonary, gastrointestinal, musculoskeletal, and burn injuries. Clinicians should also be aware of the common infections that can complicate the further care of their patients based on the geographic region where they were injured.
