| الوصف: | 日中独居要介護者の家族に対する記述ケアを実践し,看護師と家族の間に生じる相互作用(記述とその反応)のありようを明らかにして,訪問看護における家族ケアの方法として検討した。なお,記述ケアとは,ノートなどへの記述によって家族をケアする方法と仮定義した。記述方針に基づいて実践した記述ケア活動と,それに対する家族の反応を質的に分析した。対象者は5名の女性。記述方針,記述ケア活動,家族の反応の間には5名の対象者に共通した関連があった。共通した関連のひとつの例を示すと,記述方針「家族が要介護者や家族の介護について肯定的な感情を共有できる看護師の存在を認識することで情緒的に安定できるようにする」に沿って,ノートに【家族がその場にいないことを考慮した看護師の活動の表明】を記述すると,それに対して家族は[コミュニケーションが可能な関係を実感]したり,[不在の状況下でも看護師の支援を実感]していた。記述ケアという方法は,家族を精神的に支え,家族の成長を促進するという家族ケアの実践が可能であり,その特徴には,意図的にコミュニケーションの機会を提供すること,意図的にケアを「見せる」ことがケアになること,時間と空間を越えてケアすることであると考察した。訪問看護の通常のケアとして臨床に導入するためには,記述にかかる時間を短縮する工夫が必要であるなどの課題があった。 The study aimed at discussing the descriptive care for families with a sick relative living alone at home. The studyrevealed the state of interaction between families and nurses during the descriptive process. Descriptive care is a methodof caring for the family by writing notes.Descriptive care activities were conducted on the basis of description policies. And the descriptive care activitiesand the reaction of the families were analyzed qualitatively.The subjects were five women. There were some common relationships in subjects between description policies,descriptive care activities and reactions of the families. For example, a common relationship as a description policywould be that the family is able to be emotionally stable by recognizing the existence of a nurse who can share thepositive emotions with the family regarding the care. Descriptive care action includes statement of activities of nurses inthe absence of family, whereas the reaction of the family includes the realization that it is possible to communicate withthe nurses through descriptions and the feeling of support of the nurses even in their absence.The methods of descriptive care can mentally support families and promote growth similar to one of the nursingcare. Characteristics of descriptive care are as follows: to provide opportunities for deliberate communication, tointentionally ‘show' care and to care beyond time and space. Therefore, to introduce clinical care of a visiting nurse, it isnecessary to devise a method of documentation that requires less time. |
