الوصف: Uvod: Utapanje je rezultat submerzije u neku vrstu tekućine, vodi do hipoksije, acidoze, hipotermije, aritmija te multiorganskog zatajenja i smrti. Terapija kisikom glavni je tretman liječenja. Objedinjeni hitni bolnički prijam (OHBP) tijekom posljednjih godina primjenjuje neinvazivnu ventilaciju (NIV) namijenjenu pacijentima s akutnom respiracijskom insufi cijencijom. Zbog pozitivnih učinaka NIV-a na hemodinamiku i respiraciju nije nužna uobičajena invazivna strojna ventilacija. Postoje dva modaliteta NIV-a: kontinuirani pozitivni tlak u dišnim putevima (engl. CPAP, Continuous positive airway pressure) i dvofazna ventilacija pozitivnim tlakom (engl. BiPAP, Bilevel positive airway pressure). NIV se preporučuje kao terapijski izbor, a rutinska primjena kortikosteroida, diuretika i empirijska primjena antibiotika nije preporučljiva. Zbog porasta broja utopljenika liječenih NIV-om u OHBP-u istraživanjem se analizirala učestalost i učinkovitost primjene NIV-a kao glavnog terapijskog pristupa. Analizirani su i ostali podatci iz medicinske dokumentacije utopljenika. Cilj rada: Uvid u usklađenost s postojećim preporukama i smjernicama te učinkovitost NIV-a u liječenju utopljenika u OHBP-u KBC-a Rijeka u razdoblju od 1. siječnja 2017. do 31. prosinca 2019. godine. Metode rada: Istraživanjem je uključeno 98 utopljenika. Retrospektivno su analizirani podatci iz medicinske dokumentacije utopljenika u navedenom razdoblju. Introduction: Drowning is the result of submersion in a fl uid, leading to hypoxia, acidosis, hypothermia, arrhythmia and multiorgan failure and death. Oxygen therapy is the main treatment option. For the past few years, the Emergency Department (ED) uses noninvasive ventilation (NIV) in patients with acute respiratory failure. Because of positive effects on hemodynamics and respiration, there is no need of conventional mechanical ventilation. There are two modalities, i.e. continuous positive airway pressure (CPAP) and bilevel positive airway pressure (BiPAP). NIV is recommended as the main therapeutic option. Routine use of corticosteroids, diuretics and empirical antibiotics is not recommended. Considering the increased number of drowning victims in the ED that were treated with NIV, this study analyzed the trend of using NIV and its effectiveness as the main therapeutic option. Other data from medical records of drowning victims were also analyzed. Objective: The aim of this study was to inspect compatibility with the existing recommendations and guidelines, and the effectiveness of NIV in the treatment of drowning victims at the Rijeka University Hospital Centre ED in the period from January 1, 2017 until December 31, 2019.
