Introduction: Chronic patellar tendon ruptures are uncommon, and many distinct surgical techniques have been utilized for reconstruction with allograft or synthetic materials. The optimal treatment approach remains unclear in the literature. Case presentation: We present the case of a 42-year-old male with a chronic rupture of the patellar tendon that was reconstructed with contralateral hamstrings and augmentation with an ipsilateral semitendinosus tendon. Clinical discussion: Results were excellent at 6 months follow-up with the patient achieving an optimal range of motion, KOOS score of 78.2, and significant improvement in his pain. Conclusion: This technique achieves the preoperative goals of restoring quadriceps function, restoring patella anatomy, being cost-effective, and allowing early postoperative mobilization.
