| مستخلص: | Introduction: Acute appendicitis is the most frequent abdominal condition that requires surgical intervention in the world. The diagnosis is based on clinical symptoms, image findings and surgeon expertise. About one in three of the patients don´t have the usual right lower quadrant pain, explained for the different positions of cecal appendix. Left acute appendicitis is rare and it happens due a very long right appendix or in patients with congenital conditions such as situs inversus totalis and midgut malrotation. The situs inversus totalis is an extremely rare condition that afects 0,001 - 0.01 % of population (1). It´s a difficult diagnosis, and usually is incidental, when the pacient is studied with the routine exams. These patients have a 0,016 - 0,024% incidence of developing acute apendicitis, and generally present some complication. The diagnosis of surgical pathologies in patients with anatomical variations is still a challenge, and this is the reason to keep present these diseases. Methods: in this opportunity we present a case of left acute appendicitis in a patient with situs inversus totalis, without preoperative knowledge about this condition and searched about this topic in the published literature through PubMed, finding few published cases, that makes more relevant the report of this one. Results: there have been published 95 cases of acute apendicitis in situs inversus totalis, 57 were men and 38 women. Almost all the patients presented left lower quadrant pain, but a few ones presented pain in both sides. Conclusions: Left acute appendicitis is a rare condition the practitioners have to have in mind in patients with left lower quadrant pain. The images and laparoscopic interventions are useful to make an opportune diagnosis and treatment. Introducción: la apendicitis aguda es la principal causa de abdomen agudo quirúrgico en el mundo. Su diagnóstico está basado en síntomas clínicos bien establecidos, hallazgos imaginológicos claros y la experiencia del cirujano. Aproximadamente en un tercio de los pacientes no se presenta el característico dolor en la fosa iliaca derecha, debido a la variedad de posiciones del apéndice cecal. La apendicitis que se manifiesta con dolor en fosa iliaca izquierda es poco frecuente y se presenta en casos de apéndices extremadamente largas con su punta en el lado izquierdo, o como una serie de enfermedades congénitas que favorecen la localización izquierda (p.e. situs inversus total o malrotacion intestinal). El situs inversus total es una entidad extremadamente rara, que afecta el 0,001-0,01 % de la población. Los pacientes con situs inversus totalis tienen una incidencia de 0,016-0,024 % de sufrir apendicitis aguda y generalmente se presentan con alguna complicación. Métodos: se presenta un caso de apendicitis aguda en una paciente con situs inversus total, sin conocimiento preoperatorio de la enfermedad y se hace una búsqueda de la literatura concerniente al tema vía Pubmed. Resultados: hasta la actualidad se han publicado 95 casos de pacientes con apendicitis aguda en situs inversus total. En la literatura revisada se han descrito 57 casos en hombres y 38 casos en mujeres, con una edad media de 29 años. La mayoría de pacientes se presentan con dolor en el cuadrante inferior izquierdo, pero una pequeña proporción se pueden presentar como un dolor bilateral. Conclusiones: la apendicitis aguda izquierda es una condición rara, pero que hay que tener en cuenta en pacientes con sintomatología de dolor en cuadrante inferior izquierdo. Las imágenes y la laparoscopia son útiles para realizar un diagnóstico y tratamiento oportuno. |
