مستخلص: Background: Catheter-associated urinary tract infections (CAUTI) and central line-associated bloodstream infections (CLABSI) are two common and potentially preventable healthcare-acquired infections (HAI) impacting intensive care units (ICU) worldwide. CAUTIs and CLABSIs not only influence patient morbidity and mortality but also have a fiscal impact on healthcare organizations given the non-payment policies of the Centers for Medicare and Medicaid Services for CAUTIs and CLABSIs (Calderwood et al., 2018). There is ample evidence supporting the use of checklists during ICU multidisciplinary rounds as an effective tool to reduce device-related infections including CAUTIs and CLABSIs in the ICU setting (Duclos et al., 2018). At Penn-Medicine, Chester County Hospital CCH), there has been an increase in HAI rates in the years 2021 and 2022. This Doctor of Nursing Practice project centers on the implementation of a device-focused, daily ICU rounding tool intended to reduce CAUTI and CLABSI rates in medical ICU patients by 40% within a six-week intervention phase. Methods: For six weeks between May and July 2022, the CAUTI, CLABSI Rate Reduction Tool (C2R2) was completed daily on all patients in the ICU at Penn Medicine – CCH during ICU rounds. Data on CAUTI and CLABSI rates and on intraurethral catheter (IUC) and central venous catheter (CVC) days were collected and compared to infection and utilization rates six weeks before implementation of the intervention. Demographic information was collected on all patients that received the intervention.
